Office Policies

Insurance & Billing

• We accept most major insurance PPO plans.

• We accept BCBS HMO Plan / PCP must be listed as Dr. Simon Lim(not BCBS MY BLUE PLAN)

• Patients are responsible for any copays, deductibles, or services not covered by their insurance at the time of visit.

• Please bring your insurance card and a valid photo ID to every appointment.

Referrals

• Some insurance plans require a referral from your primary care physician before seeing a specialist.

• It is the patient’s responsibility to obtain referrals prior to their appointment.

Payments

• Copays and non-covered services are due at the time of service.

• We accept cash, credit/debit cards, and most HSA/FSA cards.

• Self - pay options are available

Cancellations & No-Shows

• Please notify us at least 24 hours in advance if you need to cancel or reschedule.

• Missed appointments without notice may result in a cancellation fee.
